Your scholarship award will be paid directly to the institution of your choice. In order for the Foundation to process the payment, you must submit the following form and provide proof of enrollment (i.e. letter from registrar’s office, bill, or class schedule). Once received, we will process the payment. Please allow 2 weeks for processing.
